Your First Visit to Polaris Family Medicine
We look forward to seeing you at your appointment with Polaris Family Medicine. Whether you're returning to see Leigh and Annie again or discovering our practice for the first time, we'll treat your initial visit as a new patient appointment to ensure your records are set up correctly and establish a vital foundation for our ongoing relationship with you. During your first visit, we make sure to obtain important background information about your medical history and give you time to get to know your provider. To help you prepare and understand what to expect, please review the information below and complete the following checklist prior to your appointment.

Pre-Appointment Checklist
- Change your primary care provider with your insurance company prior to your visit. Please ask your insurance company to list either Leigh Dobbs or Annie Valente as your primary care provider. If you have any problems with this, please ask your insurance to contact us at 978-637-7643.
- Please download and complete our Patient Registration Form. This will save you time at your initial visit.
- Bring your insurance card and co-payment to the visit.
- Request your medical records from your previous provider using our Record Release Form. Please complete this form and send it to your previous provider as soon as possible.
- Please complete the Release of Information Form prior to your first appointment. Only the people listed on the Release of Information Form will be allowed to communicate with our office regarding your health care.
- Please download and login to our Patient Portal app (Healow) to access your medical records, test results, and referral status. You can also access the web portal through this link. We can help you sign up for portal access when we set up your appointment if needed.
- Please download the Spruce Health app for secure communication with our office and your provider. This allows you to send messages, photos, and receive updates from your care team. You can connect with Polaris on Spruce Health through this link. We can help you register for Spruce Health when we set up your appointment if you run into any problems.

About one week prior to your appointment, you'll get an email with a link to complete all of your pre-registration questionnaires and digitally sign our Consent to Treat and Healthcare Agreement. If you'd like to complete these in person, please plan to arrive 15 minutes prior to your scheduled appointment.
